On your first visit to our office, our receptionist will ask you to complete a patient information form. The questions we ask will provide the doctors with essential data on your health and medical history.

Please be sure to bring your current insurance cards and all medications you are currently taking. There will be times when we will have to contact you, so please provide as complete information as possible.

Periodically, we will update your records, please inform us of any changes such in address, phone number, etc. New patients are requested to bring old records of previous testing, ECGS, and hospitalizations, if available, to the initial visit.
